surely this isn't the first time a customer has sent in a Tarion form from the Tarion website, is it?  That is exactly the way they print off or save.  I have no control over the resolution of the form from the Tarion website.  I can read the form when it prints.  It's pretty clear to me what the words say even though I agree they're not perfectly clear.  

Here is another copy (my third) of the form in PDF format.  This is acceptable in quality to me.  

A rejection of this copy will be unreasonable to me and I will contact Tarion directly to share my concerns, as should you about the form resolutions.
